Course Contents
The course aims to provide knowledge about the care of frail subjects and the models of transition from in to outside the hospital

Learning Outcomes
Provide the student with information useful to:
- Identify and manage frailty and chronicity;
- Understand the role of the GP in the management of frailty;
- Inform and discuss relatively to novel models for the care of frail subjeects
Prerequisites
To have a basic knowledge of the main physiopathological mechanisms and clinical manifestations of the internal medicine diseases

Assessment Methods
Oral examination with the discussion of a project work which, starting from a clinical scenario will allow the student to show the knowledges aquired about the care of frail and chronic subjects.
Detailed Syllabus
- The concept of frailty and chronicity and their care
- Identification and prention of frailty: deprescribing, prevention of falls, identification and prevention of delirium, promotion of healthy life styles
- Interactive discussion of clinical cases and ECG scenarios
- The definition of the role of the GP, with a particular focus on the care of frail and chronic patients
- Adherence to and persistence in therapy
- The concept of Team Based Care and Task Shifting
- Patient Counseling
Expected Learning Outcomes
At the end of the course the student must have learned a proper approach to the care of frail subjects.
