Course Contents
Each internship can be carried out either at the AOU Maggiore della Carità of Novara or at the Sant'Andrea Vercelli Hospital, according to the calendar provided by the secretariat. The internship is carried out in the afternoon (the schedule is approximately from 14 to 19, postponed for those who have to go to Vercelli).
The departments where the internship takes place are as follows:
1. INTERNAL MEDICINE OR EQUIPOLLENT SPECIALTIES,
2. GENERAL SURGERY,
3. NEUROLOGY,
4. CARDIOLOGY,
5. PNEUMOLOGY.
The teaching load is the following: 10 afternoons for Internal Medicine (4 days of collective exercises + 6 afternoons in the ward), 5 afternoons for cardiology and surgery, 3 afternoons for pulmonology, 2 afternoons for neurology.

NB: changes for personal reasons are not allowed, in exceptional cases it is possible to change only in a 1: 1 ratio by preliminarily sending the request to martina.aversa@uniupo.it. If there is a need to suspend the internship for reasons related to COVID, the student must communicate his absence to tirocinio.med@uniupo.it and any positivity or isolation to referente.covid@uniupo.it. In the latter case, for any request for distance lessons, the student must fill in the form available at the link https://forms.gle/DeosxCA3AWpWxAAM6.
The contents that will be carried out during the attendance in the various departments can be summarized as follows:
- Collection of medical history (theoretical revision and bedside learning with a dedicated tutor)
- Collection of medical history (practical exercise at bedside on patients selected by the tutor)
- Clinical examination (theoretical revision and bedside learning with dedicated tutor)
- Clinical examination (practical exercise at bedside on patients selected by the tutor)
- Cardiac sounds (exercise with dedicated software)
- Pulmonary sounds (exercise with dedicated software)
- Detection of blood pressure (practical exercise)
- Theoretical revision and practical exercise on basic electrocardiogram
- Guided discussion of clinical cases of hemogasanalysis, chest radiography, abdomen radiography
- Guided discussion of specialist cases (cardiology, neurology, surgery, pneumology).

Learning Outcomes
1) Knowledge and understanding.
The training objectives that are highlighted in relation to this descriptor are pursued through practical exercises in the clinical wards most characterizing medical semeiotics (internal medicine, cardiology, neurology, surgery, pneumology). The aim is to acquire the student's basic skills for a correct clinical approach to the patient. The student, in detail, acquires the following pieces of knowledge:
- knowledge of how the medical history is collected, interpreting changes in biological functions (e.g., diuresis, urination, defecation) and the main symptoms of diseases distinguished by apparatus;
- knowledge of how to perform a thorough examination;
- knowledge of how to set up the compilation of a medical record oriented to problem solving;
- knowledge of the pathophysiological mechanisms that regulate the respiratory and metabolic control of blood gases.

2) Ability to apply knowledge and understanding.
The training objectives that are highlighted in relation to this descriptor are reached through practical exercises and rotations in specific departments (internal medicine, cardiology, neurology, surgery, pneumology) providing direct under supervision student-patient interaction, clinical case simulations, and interactive discussion with students with particular concern to problem-solving and case study analysis.
In particular, the student acquires the following abilities:
- ability to establish a proper medical-patient relationship;
- ability to juggle with the basic concepts of clinical reasoning;
- ability to perfect the compilation style of a clinical record;
- ability to properly use instrumental and laboratory investigations basing on the diagnostic value of the tests (sensitivity, specificity, predictive power, the threshold concept, cost/benefit ratio);
- (initial) ability to apply the correct methodological tools to set up a rational solving problem in the medical field;
- ability to correctly interpret a blood gas analysis.

3) Independence of judgment.
Upon completion of the course the student will acquire the ability to collect, process and, whenever possible, to interpret the multiplicity of complex and/or fragmentary pieces of information arising from the analysis of the main signs and symptoms reported by the patients; the student should also become able to orderly transcribe them into a medical record.

4) Communication skills.
After completing the course the student will need to speak with a correct technical language.

5) Learning Capacity.
At the end of the course the student will be able to a) properly collect medical history; b) perform the main semiotics maneuvers; c) compile a medical record comprising medical history, physical examination and (basic) list of problems. Essentially, the student must have developed useful skills to deal with subsequent courses, with particular reference to internal medicine and clinical pathology.
Prerequisites
In order to be admitted to the clinical departments the student must have been declared fit at the occupational medicine visit and must have attended the course on safety
Teaching Methods
During the clinical rotations different teaching methodologies will be alternated, according to convenience In detail:
- Simulation of heart and lung sounds via dedicated software (purchasable under license);
- Tutoring of students at patient bedside in the collection of history and physical examination;
- Individualized correction of at least two medical records compiled by the students during their rotation in the department of internal medicine;
- Practical exercises on the main imaging techniques (radiography of the chest and abdomen), electrocardiogram and arterial blood gas analysis.

Please note that it is possible that depending on the evolution of the SARS-COV-2 pandemic, there may be changes in the aforementioned methods of conducting internships, with the enhancement of distance learning, which will subsequently be communicated to students
Additional Information
To attend the departments are needed: white coat, badge, stethoscope, gavel and torch, masks (better if FFP-2).
Assessment Methods
The clinical training will be considered passed (and the student may therefore be admitted to the exam) if the student has regularly attended the required 5 wards and completed the internship program in the various wards; at the end of the period he needs to have completed in written form at least two medical records (including medical history, physical examination, basic list of problems) of patients visited during the internship in internal medicine.
The internship form can be downloaded from the DIR page of the METODOLOGIA E SEMEIOTICA MEDICA course, which must be filled in each time at the end of each of the 5 internships provided by a tutor who has followed the student during the respective internship; this form must then be delivered to the reception of Palazzo Bellini within the following 5 working days, at the same time the student must sign in a special register.
