Learning Outcomes
At the end of the course, the student will be able to apply the knowledge acquired indesigning educational plans for the person assisted with chronic pathologies, the family andthe community. The module has the following objectives: - to know, identify and evaluate thehealth status and needs of families in their cultural and community context by recognizing theintervention priorities, planning and implementation phases of assistance to families in needof specific interventions

Teaching Methods
Lectures will be used to impart theoretical knowledge, discussion of clinical cases and care situations in order to plan educational interventions in relation to the fields being addressed.

Assessment Methods
The profit examination is proposed in a single session. The test will consist of a written test. Passing with 60% of the correct answers in each discipline will yield a grade expressed in thirtieths that will average with the oral test in Community Nursing and Therapeutic Education and Nursing in Mental Health and Psychiatry.
Detailed Syllabus
Territorial structure (DM 77/2022), the network of territorial services, diversification of care and continuity of care. Networks: formal, informal, primary and secondary networks and the third sector.
Stakeholders in the care system: social, health and caregivers.
Framing of family and community nursing.
Clinical and social frailty assessment, family and resource assessment.
Person intake and interventions in ambulatory, home and residential care. Case discussions.
Chronicity management in the community (person education; self care and self management). Discussion cases.
ASL health promotion campaigns and communication in the digital age: road and domestic accidents, occupational injuries, lifestyles and chronicity prevention, addiction and sexually transmitted diseases.
School nursing and therapeutic education with adolescents. Case discussions.
Expected Learning Outcomes
Ability to apply knowledge and understanding: - Analyze the main educational models,highlighting their theoretical approaches, aims and objectives, types of intervention, conditionsand factors of learning. - Plan a simple educational process by arguing educational objectives,methods and strategies. Judgment autonomy - Analyze an educational curriculum accordingto the main theoretical approaches - Design an educational intervention on the basis of"planning for objectives" Communication skills: - Approach educational issues with mastery ofthe specific language in order to develop educational planning for individuals, groups andcommunities.
