Learning Outcomes
The course aims to provide the student with the tools and knowledge to promote and guarantee the care of the local patient on the basis of current legislation and the structures/services/networks present in the territory.

Frontal lessons with discussion between students and teachers on the topics covered in the classroom.

Guided study and discussion of clinical cases in group work.

Assessment Methods
Oral test on a clinical case in which the candidate is asked to define the treatment/assistance path and the process of activating the territorial network based on Ministerial Decree 77/2022.

Expected Learning Outcomes
The student, on the basis of the knowledge acquired, will be able to define the treatment paths that can be activated to ensure appropriate care of the person with chronic pathology.

They will be able to plan an individual care program, identifying the greatest care needs and evaluating the possible strategies to be implemented to guarantee the best treatment path.

Will develop skills to define and argue the planning of the individual care plan.
