Learning Outcomes
The course aims to provide the students with the tools and knowledge to promote and guarantee the care of the local patient on the basis of current legislation and the structures/services/networks present in the territory.

Teaching Methods
Frontal lessons with discussion between students and teachers on the topics covered in the classroom.

Guided study and discussion of clinical cases in group work.

Assessment Methods
Oral exam on a clinical case, chosen from a pool of thirty, in which the students is asked to define social care needs, individual care planning, and the process for activating the local care network using the structures provided for in Ministerial Decree 77/2022.
The grade is expressed in thirtieths.
The exam is 15 minutes long.

The regulatory reform of territorial assistance, The new territorial healthcare based on Ministerial Decree 77/2022, The Territorial Operations Centers and the territorial structures, Telemedicine and home care: the role of technologies, professionals and patients, Family and community nurses: role and levels of intervention, Home care: taking charge of the patient in the various types of care, The palliative care and pain therapy network, The importance of caregiver empowerment and self-efficacy, Local assistance to the frail patient and the chronic patient, Territorial assistance and socio-health performance: expected care vs. care provided.
Expected Learning Outcomes
The students, on the basis of the knowledge acquired, will be able to define the treatment paths that can be activated to ensure appropriate care of the person with chronic pathology.

They will be able to plan an individual care program, identifying the greatest care needs and evaluating the possible strategies to be implemented to guarantee the best treatment path.

Will develop skills to define and argue the planning of the individual care plan.
