Learning Outcomes
Acquisition of integrated approach for the correct programming and planning of treatment path in the community setting, according to clinical picture and care needs of patient
Prerequisites
Basic knowledge of the main epidemiological aspects, biological and pathophysiological mechanisms, clinical manifestations and healthcare needs in relation to the context of the disciplines of the integrated course

Assessment Methods
Developing of a project work which, starting from a clinical scenario, will allow the student to show the knowledges aquired about the care of frail and chronic subjects. The clinical case will be drawn at random, during the exam, from a pool of cases previously made available to the learner

Expected Learning Outcomes
Knowledge of main healthcare models and structure of the local healthcare network for management of frail individual and chronically ill.
Management of health pathways in the community setting and achievement of tools for their implementation in the diagnostic, therapeutic and healthcare fields.
